Routine appointment requests can be made through the E-Consultation service as detailed below, please do not call the surgery to request these appointments.

For results of investigations such as blood tests and x-rays etc, please phone after 11am or check the NHS app.

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

E-Consultation

E-Consultation is a digital triage system that allows patients to be able to send in a triage request to the surgery, this can be for symptoms that you are having, a prescription request, or even for an administration request.

 You can access this service through the NHS app, the web on https://accurx.nhs.uk/patient-initiated/F81222 , and via our website by clicking on the link on the home page and follow the form through to the end, answering the questions as appropriate to your request/needs, then submit the form.

 

Home Visits

Patients are requested to telephone before 10.30am, Monday to Friday, if a visit is required, unless an emergency arises later in the day. The receptionist will need as much information as possible to enable the doctor to prioritise requests. 

 

Weekend and Night Cover

The practice telephone operates 24 hours a day. After hours and at weekends you will be advised to contact NHS 111, operated by IC24, who cover us when we are closed, jointly commissioned by the four Clinical Commissioning Groups in South Essex.

 

NHS 111

In addition to the services provided by the practice, NHS 111 is available 24 hours a day for confidential medical advice and for direction to the most suitable service. Calls to NHS 111 service are free from both landlines and mobiles.

 

Extended Hours HUB Service

An extended hours HUB service is offered by BB Healthcare Solutions. This service offers patients the opportunity to see a GP, nurse or clinician after the surgery has closed. Appointments can be pre-booked by contacting the reception team at Queens Park Surgery. This service operates Monday to Friday from 18:30-20:00, and 09:00-17:00 on Saturdays.
